Glake89 was correct in that he showed a pic of mode selection actuator, however it may not be the correct actuator for the job thunder cad mentioned. The blend door is under the dash. You will have to disconnect the negative side of the battery before you start. Then remove any silencers or covers. The dash will not have to be removed. The blend door actuator is located on the left side of the HVAC unit under the dash.

The blend door actuator (motor) tested good, but the shaft it connected to on the heater box was loose inside the heater box. When turned, the shaft sticking out would catch a little bit, but then would continue to turn a round and around, it should only turn a quarter turn or so.
